By scheduling and participating in services with Spirit of Nature Holistic Medicine Practice, you acknowledge that you have read, understood, and agree to the following terms:

The holistic health assessment is strictly for educational and informational purposes and is intended to support overall wellness and serve as a self-help tool.

The consultations, wellness recommendations, and holistic health interventions provided by Spirit of Nature Holistic Medicine Practice are educational and supportive in nature. Our goal is to help you explore natural and integrative approaches to wellness. These services are not a substitute for medical care provided by a licensed physician or other qualified healthcare provider.

The assessment is not medical advice and should not replace professional medical advice, diagnosis, or treatment.

  • The practitioners at Spirit of Nature Holistic Medicine Practice are not acting as your primary medical provider. Spirit of Nature Holistic Medical Practice acts as your teacher as doctor and does not offer medical advice, but suggestions and education for your holistic healing.

  • We strongly encourage you to maintain a relationship with a licensed healthcare provider for ongoing medical care, diagnostic testing, medications, and emergency need.

4. Limitations of Services

By signing our informed consent before recommendations are offered (or continuing with services online), you confirm that you have read and understood this Informed Consent & Terms document, and you agree to hold harmless Spirit of Nature Holistic Medicine Practice, its practitioners, and affiliates for any adverse outcomes related to your participation.

7. Electronic Consent

Your electronic signature or continued use of services constitutes the same legal acceptance as a physical signature on paper. This will be signed before a Holistic Health Consultation will be conducted. This practice is accordance with our licensed provider for alternative health practices PWA and are in accordance with their rules and regulations.
